Termination and Follow-Up
Social workers are expected to understand the length of time that therapy, services, and counseling are provided to the client. At the end of the service period, termination occurs. In this stage of the generalist intervention model, the client-social worker relationship concludes and the client is often expected to continue on as a self-reliant advocate, based on the intervention plan. It is important to prepare and empower clients for termination by referring to this time and date and by supporting understanding and acceptance of this stage in the intervention process.

Discuss how your site uses follow-up with clients after termination. Keep in mind, follow-up is the stage where the social worker checks in to learn of the continued effectiveness of the intervention plan and assess ongoing progress.
We are currently in the process of terminating a client in July 2022. Hence, we have begum the termination process with the client. We began the termination process in January which gives us 6 months to prepare the client for the transition. We are helping the client to transition
into the new environment. We are giving him skills that he can use to carry him throughout life. We are reaching goals that are on his intervention plan that will allow him to be more sociable and to boost his confidence. We want the client to embrace being an independent adult that can survive in society, I will not be at the center for the complete termination. However, I have seen the exit interviews and discussed how they will be implemented by the Qualified Professional. It appears the client is accepting the transition and assisting us with getting him transitioned to a new facility. The client will be going to a facility that two of the current employees also work with. We will gather information from the staff of his progression or regression. Also, we will speak with the staff at the new center and provide them with information that we think will be helpful in him becoming adjusted to their facility.
